same problem, have tried: Release and re create, and it is working in my case:

      this->pVoice->Release();  // cancel the show
      hr = CoCreateInstance(CLSID_SpVoice, NULL, CLSCTX_ALL, IID_ISpVoice, (void**)&pVoice);
