Solved by changing file "mm.txt" to:

$root /home/test/my_dir    
std     
my_module    
./my_headers.hpp

and locating "my_headers.hpp.gcm" file in

"./,/my_headers.hpp"

Note: the comma directory in "./,/my_headers.hpp" is not a typo, it is needed.
