The connection issue in otrs for smtp could be because of wrong smtp authentication issue.
check your smtp config settings.

What helped me.

Go to ...
System Configuration -->> search for sendmail --> make sure that the authentication type, authentication user (this should be the system email address you are using) and authentication password (this should be the correct password to that email ) is correct.
